At Southgate we feel like continuum of care is important for the health and wellbeing of the seniors in or area. 

Our independent living facility offers services and amenities that you won’t find at most other facilities like ours. We strive to extend our residents’ ability to live independently as long as possible.  Concierge-style therapy services including onsite physical, occupational and speech therapy help us to bridge the gap between independent living and skilled nursing care.

When independent living is no longer possible, long-term care is the natural next step, and our residents are able to seamlessly make that transition to our skilled nursing facility.
